Reports to:

Head nurse &House nurse’s supervisor &ACNO

Accountable to:

ACNO & CNO

Job Summary:

Medication nurse duty assignment is performed as a means of ensuring the safe administration of therapeutic medication to patients in accordance with all legislative and regulatory requirements.

Responsibilities and duties:

1-      Keep the medication room clean, tidy, and well organized.

2-      Keep the medication room door locked all the time.

3-      Keep the narcotic key with her during the duty hours, make a daily count with the out going.

4-      Allow only the authorized people to get in.

5-      Review written prescription for the medication.

6-      Prepare medication as per the written prescription.

7-      Has the medication checked as per the requirements.

8-      Correctly identify patient.

9-      Check prescription with second RN to verify the following;

10-  Right patient's Name, Medical record NO. Room NO. Bed NO.

11-  Right Medication Name

12-  Right Dose, Concentration, Additives, Preparation, & Rate

13-  Right route & Direction

14-  Frequency date, time Frequency, & Duration

15-  Right approach & Manner

16-  Right documentation, Recording, & Reporting if any changes

17-  Check for any notification of medication allergies/adverse reactions

Assure and follow up that the assigned nurse will:.

18-  Explain Procedure to patient.

19-  If the patient refuses to receive his medicines , then the nurse is required to:

20-  notify the doctor who prescribed the medication;

21-  Document the outcome.

22-  Remain with patient until patient swallows medication.

23-  Immediately after administering the medication, document the administration and sign in the patient's medication sheet.

24-  Monitor and record the patient’s response to the medication.

25-  If the patient starts to exhibit side effect or problems after the administration of the medication, the prescribing doctor is to be notified immediately.by he assigned and medication nurse

26-  Place patient’s ID label or write clearly patient data on the addressograph section or patient name and medical record number on the MAR.

27-  Write date for administration for each medication order.

28-   Patient’s weight shall be written in the space provided.

29-  Use a ballpoint pen and press firmly.

30-    Medication nurse shall assure that all orders must be signed, dated and / or stamped by the physician.

31-  Weight-based dosing, relative to safe and effective dosing for pediatric patients: the medication nurse shall verify with the assigned nurse

32-  The patient’s weight

33-  The patient’s age

34-  The patient’s physical condition

35-  The medication nurse shall verify the medication order as per Medication order verification/clarification policies

36-  The medication nurse shall call the physician and will follow the requirements when medication orders are incomplete, illegible or unclear as per the medication order clarification policy before preparation or administration of the medications. 

37-  Medication nurse verify each medication will be individually packed and labeled with medication name, expiry date and the patient identification label shall be affixed.

38-    Medication nurse shall  minimum possible quantity shall be dispensed to the nursing unit.

39-  The medication nurse will verify the dispensed medication with the medication order.

40-  The medication nurse will bring the medication to the nursing unit and store it in the individual patient medication cassette

Educational Responsibilities:

Under the direction of the education team the nurse will:

1-      Contribute to the identification of training needs pertinent to the clinical area.

2-      Participate in the clinical induction of all new nursing and support staff.

3-      Develop leadership ability in order to act as an effective role model.

4-      Assume responsibility for own learning and development needs.

5-      Attend all required in-services training programs.

6-      Keep up to date with mandatory training such as Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) if possible.

Communication and Interpersonal skills:

1-      Follow the proper channels of communication according to the department organizational chart.

2-      Must be able to verbally and in writing of communicate effectively.

3-      Maintain professional appearance and dress code.

4-      Wear Identification badge when working.

5-      Comply with guidelines and protocols.

6-      Must be innovative and have critical thinking skills.

7-      Always promote good rapport in the unit to maintain teamwork.

Qualifications:

  Education:

·       B.S.N.

Experience:

·       3 years’ experience or Critical Care nurse is preferred.

Training:

Must be registered with the Egyptian Syndicate of Nurses and / or professional registration body of origin.

·       5o hours of training per annum mandatory.

·       BLS certification is Obligatory & ACLS is preferred.

Dress Code:

According to hospital Policy.

Competencies:

     1-    Aseptic Technique.

     2-    Assessment & Reassessment.

     3-    BLS.

     4-    Code Blue.

     5-    Vital Signs.

     6-    I.V Cannulation.

     7-    Medical Equipment.

     8-    Medication Administration: Oral, IV, Nebulizer, IM, SC, Topical, PR, PV.
